/give <player name> <item identifier>{CustomModelData:<custom_model_data value>}
Examples :
/give @p diamond_sword{CustomModelData:4901} – gives Rocket Power Sword to a player. Also try bow, diamond_axe, diamond_pickaxe, etc…
