Fix player heads not loading in mc 1.21+

• Fix item stack giving a stack of 99 instead of 64 for 1.21
• Fix issue with effects in 1.20.5/1.20.6

Fix reward messages that were not being sent to the player.

This update adds a menu to execute and run crate rewards.
