- Additional fixes to /phead when getting slim skin models

- Fixed issues with UUID heads used in custom categories (they should now be converted to base64 heads)
- Fixed issue with /phead not working correctly for the same reason as the UUID heads.

setOwnerProfile seems to be broken in the API

Added 308 more heads to the database, use /hdb r to reload the database. Regular updates provided by http://minecraft-heads.com

It appears my last patch didn't fully fix the issue with the "Failed to initialize head reflection, are you using a supported server version?".

This patch should address that issue.

Fixed issues with heads being illegal items which can cause client disconnects. (Internal name is now "HeadDatabase" instead of "Head Database")

Existing heads might still cause issues though, so I will have to do some additional code to replace all existing HDB heads to the new ones.
